ELIGIBILITY REQUIREMENTS
To be eligible, candidates must be US citizens and underrepresented minority students currently in their 2nd or 3rd -year at an accredited US medical school (MD or DO). Scholars must be attending medical school in one of the following Aetna Foundation-targeted regions: Southern California (particularly Los Angeles), Connecticut, Illinois (particularly Chicago), New Jersey, New York City, and Philadelphia. Please note that the APPLICATION PROCESS IS NOW CLOSED FOR ILLINOIS.
Candidates must demonstrate: outstanding academic achievement as evidenced in academic transcripts, faculty evaluations, receipt of special academic or national medical society honors, fellowships, and awards; leadership as indicated by active participation in international health, community-based activities or initiation of innovative projects; and potential for distinguished contributions to medicine as indicated by participation in research, publications or unique clerkships.

APPLICATION REQUIREMENTS
The application is contained within the online application form. Please complete the entire form and include all uploads and attachments as required. Applicants must submit information in all required fields or the application will not be formally submitted.
Applicants are required to upload the following documents:
- Verification of Citizenship: All applicants are required to provide proof of citizenship
- Acceptable forms of proof of citizenship are one of the following:
- A US birth certificate
- A current US passport
- USCIS certificate of naturalization
- Voter's registration card (accompanied by a picture ID)
- Verification of Ethnicity: All applicants are required to provide proof of ethnicity.Some of the items listed above may list an individual’s ethnicity. Applicants may also submit parental documents indicating the applicant’s ethnic heritage
- Financial Need Information
- Applicant's Tax Return (or W2, 1099 or verification of other income if not filing)
- Parents' Tax Return (or W2, 1099 or verification of other income if not filing), applicable to dependent students only (see application form for more details)

- Documentation of Loan History (current and undergraduate) from the National Student Loan Data System & any other record of educational loans received. Please visit: http://www.nslds.ed.gov/nslds_SA/ to access your records
- Curriculum Vitae/Resume
- Brief Biosketch (200 words): A sample of the biosketch is provided here
- Official (current) Medical School Transcript
- Verification of Medical School Enrollment
- Personal Statement (500-1,000 words) on what leadership means for physician-leaders with details on health leadership activities to be continued during the scholarship period

SELECTION PROCESS
NMF objectively reviews each of the applications based on the candidate's credentials and financial need. Applicants are reviewed on the basis of letters of recommendation, financial need, personal statement, and community service and leadership activities. A rigorous application process ensures that assistance is provided to applicants with the greatest documented need and strongest commitment to primary care and public health.

NUMBER OF AWARDS AND expectations
This program funds ten (10) $5,000 scholarships.
All awardees are required to attend an NMF Gala* in their region:
- Chicago – November 2012
- New York City - April 2013
- Los Angeles – June 2013
Awardees must commit to providing annual updates to NMF about their progress for five years after their participation in this program ends.

Connecticut, New Jersey, New York City and Philidelphia applicants will be notified of a final decision in April 2013. Los Angeles applicants will be notified of a final decision in May 2013. Please do not contact NMF about application status.
